Basically, the compiler must add debug information to the executable. They type of debug information depends on the compiler, the debugger to be used, and the output format.

For example, gdb will take ELF, or COFF debug information. A debugger that loads hex files can take debug information from motorola SREC. Most source debuggers handle specific types of debug information, and compilers (on a given platform) are created to produce files with that debug info.
